The screw cap came off leaving the capo on the guitar, no matter how much I tried the screw cap didn't fit back again!

After only using this capo 5 or 6 times, it's buzzing on the lower strings. It never fit my Classical Takemine (even though the neck is pretty standard for a classical) as well as it should. But initially, I got it to work. Now, no matter how much I tighten the screw, it leaves a buzz. You can see how the surface that's supposed to clamp onto the strings is heavily worn.
